Already have a hospital Patient Portal account?

If you would like to access a summary of your Lake Chelan Hospital electronic healthcare record (EHR), please contact the patient registration desk on the 2nd floor of the hospital and ask for a link to create an account. An email is required.

After you register or are admitted / discharged from the hospital,  you will receive an email directly from Evident, our Electronic Health Record software system, from the email address

This email from “donotreply” will provide you with  instructions of how to set-up your personal Username & Password for your  Patient Portal account.
