Community Health Needs Assessments

A Community Health Needs Assessment (CHNA) is a systematic process involving the community to identify and analyze community health needs. The process provides a way for communities to prioritize health needs, and to plan and act upon unmet community health needs with resources available.

The health status of the communities we serve are assessed every three years and help us gauge our efforts for building a healthier community and developing an implementation plan to get there.
