We are dedicated to keeping your medical record information confidential. To request a copy of your medical records from one of our two locations, you will need to fill out the Release of Healthcare Information form below. Please make sure to fill the release of healthcare information form out completely and answer all questions, check all applicable boxes etc.  keep in mind that there may be a charge for obtaining your records.  Your physicians or specialists can contact us directly for records they need. Release forms may also be picked from Patient Access at the clinic or hospital. Once we receive your request form, we will send (within 14 days) the records you have selected to the RELEASE TO: name and address as stated on the authorization form.

BIRTH/DEATH CERTIFICATES

Babies born at Lake Chelan  Hospital will receive a complimentary birth certificate. While this is not a legal document, it offers proof of birth at Lake Chelan Hospital and serves as a birth memento. Legal birth certificates are available through the Chelan Douglas Health District.
