We are dedicated to keeping your medical record information confidential. To request a copy of your hospital or clinic medical records, please fill out a Release of Healthcare Information form below. Please make sure to answer all questions, check any applicable boxes and sign form.

• Keep in mind that there may be a charge for obtaining your records.
• Your physician(s) or specialist(s) may contact us directly to obtain needed medical records.
• Release forms may also be picked at the Patient Access (check in) desk at the hospital or clinic.

Once we receive your request form, we will send (within 15 days) the records you have selected to the RELEASE TO: name and address as stated on the authorization form.

BIRTH/DEATH CERTIFICATES

Babies born at Lake Chelan Health Hospital receive a complimentary birth certificate. While this is not a legal document, it offers proof of birth at Lake Chelan Health Hospital and serves as a birth memento. Legal birth certificates are available through the Chelan Douglas Health District. Please visit their website for further information and requests.

Chelan-Douglas Health District’s Vital Records Office can provide official copies of birth certificates for anyone born in Washington State from 1907 to the present.
